Out of Country Waiver

Any and all players travelling out of Canada for a tournament, exhibition, or travel game must have their parent complete and sign the Out of Country Waiver.

Hockey Canada insurance covers a maximum of $5,000.00 for out of country hockey related incidents. This coverage only applies to the registered CYO player who is playing in an event outside Canada. All other families’ members are not covered by this insurance.

This organization recommends that if you plan on attending any out of country games/tournaments you should ensure you have adequate coverage through your personal insurance policy or purchase additional private insurance for these events.

Completed forms are to be collected by the team trainer and handed in to the office BEFORE you leave for the tournament.
