Moving or changing addresses within the CYO's boundary

Even if you have moved within the same city, the Alliance requires you to submit a 2019_Residential_Move_Form. When you register for the season, you can update your address your Hockey Canada account, however the CYO will still need a change of address form filled out and submitted before Hockey Canada will authorize the player to be rostered to a team. Please fill out the form and either bring it to the office during office hours, or email it to [email protected].

Moving to the CYO from another hockey center in Ontario

If you are joining the CYO from another hockey center within Ontario, the Alliance requires you to submit an 2019_Transfer_for_Residential_Move to the association you are LEAVING. It must be filled out and submitted back to them. They will submit it and copies will be forwarded to the CYO so you can register with us.

Moving to the CYO from outside Ontario

If you are joining the CYO from outside Ontario, you will need to complete an 2019_Interbranch_Transfer_Form from Hockey Canada. Please fill in the form and either bring it to the office during office hours, or email it to [email protected].
