By submitting this form, the player and parent(s)/guardian(s) or representative of the registered player hereby agree to abide by CYO Minor Hockey and Alliance Hockey's Code of Conduct, By-Laws, Playing Rules, and the City of St Catharines' Rzone policy.
Cost for individual tryouts will be $100 total, covering the 3 sessions, and payable to CYO Minor Hockey before any player will be allowed to step on the ice.
