This form is used to collect registration information for a player who is waiting for a transfer to be approved. For players coming from out of town or from another league, they may be blocked from the on-line registration system, this form will capture their information so we can record them in the system once the transfer goes through.

The information collected will be used to add you to the St. Catharines CYO Minor Hockey registration system AFTER you have been successfully transferred through the Hockey Canada system. If you are coming to us from another league, the on-line registration system will require a transfer be approved from your previous centre before we can add you to the CYO membership.

If your transfer request is unsuccessful, we will contact you for more information so we can try to get the transfer through.
