By submitting this form, the player and parent(s)/guardian(s) or representative of the registered player hereby agree to abide by CYO Minor Hockey and Alliance Hockey's Code of Conduct, By-Laws, Playing Rules, and the City of St Catharines' Rzone policy.
Players also understand and agree that they are required to participate fully with their CYO house league team in addition to the practices and games for their select team.
Cost for individual tryouts will be $60 total, covering the 3 sessions, and payable to CYO Minor Hockey before any player will be allowed to step on the ice.
